kirItinam gadhinam chakrahastham ichchAmi thvAm dhrashṭum aham thathaiva |
thEnaiva rUpENa chathurbhujEna sahasrabAhO bhava viSvamUrthE ||

sahasrabAhO! – Oh one with unlimited arms!
viSvamUrthE! – Oh one who has the whole universe as your body!
thathA Eva – as before
kirItinam – wearing one crown
gadhinam – holding a mace
chakrahastham – as holding the chakra (disc) in your hand
thvAm – you
aham – I
dhrashtum ichchAmi – desire to see
thEna Eva – as before
chathurbhujEna rUpENa – having a divine form with four divine hands
bhava – please assume

Simple Translation (based on puththUr krishNamAchArya swAmy’s thamizh translation)

Oh one with unlimited arms! Oh one who has the whole universe as your body! I desire to see you as before, wearing only one crown, holding a mace and holding the chakra (disc) in your hand; please assume the divine form with four divine hands, as before.

Fain would I see You in that four-armed Form, O many-Armed! Be You, O Kosmic Form! That, with crown and club and discus in the hand.’

That same old Form do I wish to see, —the Form with diadem, mace and wheel borne by You in Your hands,— the Form with four arms. Assume that old Form of four arms, O (now) many-Armed! O (now) Universal Form!

Simple

kirīṭinaṁ gadinaṁ cakra-hastam
icchāmi tvāṁ draṣṭum ahaṁ tathaiva
tenaiva rūpeṇa catur-bhujena
sahasra-bāho bhava viśva-mūrte

‘Fain would I see Thee in that four-armed Form, O many-Armed! Be Thou, O Kosmic Form! That, with crown and club and discus in the hand.’
