susamskruthE – well decorated (with SEshathvagyAnam (knowledge about ones servitude, detachment from worldly aspects, etc)
gyAnakarmAthmikE nishtE – gyAna yOgam (the path of pristine knowledge) and karma yOgam (the path of following scriptural ordainments)
yOga lakshyE – to attain yOgam (Athma sAkshAthkAram – self realization)
AthmAnubhUthi SidhyarththE – (and further) to attain AthmAnubhavam (blissful experience of self)
pUrva shatkEna – In the first hexade (six chapters)
chOdhithE – ordained

The first hexade (six chapters) ordains karma yOgam and gyAna yOgam which are decorated (with knowledge about one’s servitude and detachment towards worldly matters) to attain self-realization and blissful experience of self.

madhyamE bhagavaththathva yAdhAthmyAvApthi sidhdhayE |
gyAnakarmApi nirvarthyO bhakthiyOga: prakIrthitha: ||

bhagavath thathva yAdhAthmya avApthi sidhdhayE – to attain true experience of the supreme entity named bhagavAn
gyAna karma abhi nirvarthya – that which develops from karma yOgam which is supported with gyAnam (true knowledge)
bhakthi yOga: – bhakthi yOgam (the path of devotion)
prakIrthitha: – is explained

In the middle hexade, bhakthi yOgam which develops from karma yOgam that is supported with gyAnam (true knowledge) that leads to attainment of true experience of the supreme entity named bhagavAn is explained.

ithyAdhi: – in the process through which these are attained, etc
